Plastic bronchitis is characterized by the formation of bronchial casts with branching that imitate the anatomy of the bronchial tree. It is associated to primary pulmonary disease and cyanotic congenital heart disease, especially after corrective surgery. The mechanism of production of these casts is unknown. Pulmonary lymphatic abnormalities associated with a drainage alteration named as "endobronchial lymphatic leak" has been proposed as one of the possible mechanisms. Symptoms include productive cough, dyspnea, fever and wheezing. The diagnosis is based in interrogatory, Rx and CT, and pulmonary functional tests and it is confirmed by spontaneous expectoration of casts or their visualization by flexible bronchoscopy. Treatment is still limited and it should be directed to treat the underlying disease, including removal of bronchial casts. Patients with cardiac abnormalities present highest mortality rates compared to children with plastic bronchitis associated to other diseases. We describe a 4 years old child with plastic bronchitis associated with restrictive miocardiopathy.
