Septic shock in pediatric patients may cause severe hypotension, refractory to adequate volume resuscitation and high dose catecholamines. Outcome in this group of patients is rather poor. In this scenario, alternative therapies directed to reverse vasodilatation, could help in the treatment of systemic hypotension. Terlipressin is a synthetic vasopressin analog with a longer half life that has been successfully used to treat esophageal variceal hemorrhage. It has higher affinity for vascular receptors than vasopressin, with a potent vasoconstrictor effect in animal models, as well as in adults with norepinephrine resistant septic shock. The administration of terlipressin in pediatric septic shock patients resistant to norepinephrine, has been recently published. The aim of this report is to communicate the administration of terlipressin to a child with a meningococcal septic shock resistant to catecholamine therapy.
