The eventual co-infection of R jiroveci with bacterial, fungal and parasitologic respiratory pathogens in 50 respiratory secretions, obtained by bronchoalveolar lavage, was evaluated. The samples belonged to AIDS patients treated at the Respiratory Intensive Care Unit of Muhiz Hospital. They were consecutively processed between 20/11/08 and 1/7/10 in different laboratories of Muhiz Hospital, resulting all of them positive for the presence of Pneumocystis jiroveci by microscopy in this Laboratory. The results obtained revealed co-infection in 7 (14%) of the evaluated samples, these values being lower than those expected, probably on account of the impaired immunologic status of the patients evaluated. Staphylococcus aureus and Acineto-bacter sp. were the bacterial co-pathogens more frequently isolated; while Cryptococcus neoformans was encountered as the unique fungal co-pathogen. Neither Mycobacterium tuberculosis nor other mycobacterial or parasites were identified in the evaluated samples. Empiric antibiotic treatment, the absence of studies to detect viral pathogens, and inadequate handling of the samples could have decreased the number of co-infections detected. The co-infection of JP jiroveci with other respiratory pathogens must be taken into account at the time of evaluating an adequate anti-infective treatment in AIDS patients with respiratory disease.
