Introduction. The acute respiratory distress syndrome (ARDS) is an entity that causes the highest number of lethal cases and severe morbidity in patients admitted to pediatric intensive care units. The current treatment of this condition is based on mechanical ventilation with low volume and the application of positive end expiratory pressure (PEEP). A high number of patients continue with hypoxemia in spike of the adequate application of these measures; this has resulted in the use of therapeutic options aimed at improving oxygenation. One of these is the prone position ventilation. Objective. To review the main mechanisms through which the prone position acts, as well as its effects in patients with ARDS. Conclusions. The ventilation on prone position is a secure and efficient technique to improve oxygenation in approximately 60 to 70% of patients with ARDS. It has proved to be an useful tool in the current ventilatory strategies and must be repeated daily, at least in responder patients. The lack of worsening in gas exchange and hemodynamics in non-responder patients must stimulate subsequent trials in these patients. It has not demonstrated a decrease of mortality in ARDS patients.
