Objective To describe the technocal details of three-pieced orbitozygomatic approach. Descripción. Soft tissue exposure does not vary too much compared with the classical frontopterional approach. The osseous resection is made in three pieces: 1) two cuts, one anterior and one posterior, over the zygomatic arch. Then, the arch is reflected down, attached to the masseter muscle; 2) classical fronto-temporo-sphenoidal craniotomy; and 3) osteotomy of the orbital rim and roof and lateral wall of the orbit. Conclusion. Modifications described in the present paper show the technical remarks of the three-pieced orbitozigomatic approach, with good brain exposure and excellent cosmetic results.
