Introduction. Group A streptococci are implicated in a variety of infectious diseases that include toxic shock. They are one of the complications associated with varicella. Case report. A 2 ^1/2 year old female was referred to PICU with shock. She presented varicella 30 days before admission. Fourteen hours before admission she had attended to the emergency department with fever, pain and functional impotence of her left leg. The girl was in good clinical conditions, afebrile, with preserved active motility in her left knee, though limited by pain. Bilateral hip X-rays, as well as ultrasound scans of hips and knees were normal. The patient was discharged after excluding osteoarthritis. The girl attended later to another institution, where similar findings were found. She was admitted to the emergency room in shock. Endotracheal intubation was performed, and antibiotics, volume replacement and inotropic agents were indicated. The patient died without response to treatment. Group A Beta-hemolytic streptococcus was isolated in two blood cultures.
