Ischemic stroke in childhood is an infrequent entity; in 50% of the cases its cause is unknown. Nevertheless, with the new diagnostic tools, other conditions that predispose to stroke have been identified. We present a 9 year-old girl with left acute hemiplegia with poorly defined neuroimaging studies, in which the virological examination of cerebrospinal fluid revealed enterovirus RNA. Enteroviral focal encephalitis is an infrequent neurologic entity in pediatrics. A vasculitic process secundary to viral infection has been implied, which would lead to cerebral ischemia. However, this etiology is rarely considered. We should include enterovirus in the differential diagnosis in a case of acute infantile hemiplegia.
