Seizures are a common disorder in pediatrics, occurring in 3-5% of children. There are several causes, including those of metabolic nature. We examined an 11 month-old patient referred from Santa Rosa, La Pampa province, because of convulsive status under study. She was being treated with three anticonvulsants, ceftriaxone and acyclovir, with no abnormal findings in the electroencephalogram and magnetic resonance imaging of brain. The physical examination, showed enamel hypoplasia, pallor, and mild hypertonia of lower limbs. On admission, lab tests showed normocytic-normochromic anemia, hypoglycemia, hypocalcemia, hyperphosphatemia, and low PTH levels. Brain ultrasonography: bilateral thalamic calcifications, electrocardiogram with prolonged QT. After being diagnosed with primary hypoparathyroidism, treatment with endovenous calcium gluconate, calcium carbonate as phosphorus chelating agent, and 1.25 dihydroxycholecalciferol (calcitriol) -25 mcg/day- was initiated. Tests to detect other glandular deficiencies were performed. We want to highlight the importance of considering these infrequent causes of convulsive syndrome to begin an early treatment, thus avoiding complications and sequelae.
