A 5-month old infant with respiratory distress syndrome with torpid outcome is presented. An anteroposterior and lateral chest Rx was performed and a big and heterogeneous mediastinal mass was detected. The study was completed with thoracic ultrasound and CT scan with and without contrast. This method was the most useful for establishing a pre-surgical presumptive diagnosis of teratoma. Markers of tumoral activity were all negative. The patient underwent a complete surgical resection of the tumor and partial resection of the normal thymus. Postsurgical outcome was good. The diagnosis of immature teratoma was confirmed by the pathologist. The objetive was to present a very unusual cause of respiratory distress in infants, as well as to highlight the scarce and unspecific symptoms.
