The problem with venous thrombosis in neonatal period is the limited and controversial information on therapeutic options. Therapeutic options include supportive care, anticoagulation, thrombolisis, and in a lower number of neonates, surgery. Over 80% of cases of thrombosis are related to the presence of an indwelling central line and this constitutes a risk factor for right atrial thrombi. We report the case of an extremely low birth weight infant in whom the treatment of choice for a right atrial thrombi was its surgical removal, with cardiopulmonar by pass and deep hypothermia.
