A wide variety of human chorionic gonadotropin (hCG) related molecules, which are the product of hormones synthesis and metabolization circulate in biological fluids (molecular heterogeneity). The proportion of the different molecular forms of hCG varies in sera samples of patients under different clinical situations. The results of hCG dosed in sera samples corresponding to patients during follow-up of Gestational Trophoblastic Disease (ETG) with hCGStat and hCG+b, Elecsys2010; hCG ACS-180; hCGTotal AxSYM, hCG Access against hCG Immulite have been compared. Significant differences between hCG results were found (p < 0.0001). The mean relative bias of all the samples results from a patient (DRPp) in the different immunoassays was from (-46%) to (+76%), and the relative bias of samples results of the same patient obtained by each method (DRPm) against Immulite was from (-82%) to (+105%). Important differences for the same method in different samples during the same patient's follow up were found. In conclusion: the significant differences between the methods could be attributed to the molecular heterogeneity of hCG during ETG follow-up, to the specificity of the methods, their standardization and calibration. Therefore, hCG monitoring must be made with the same method.
