Abstract

The finding of capsulated yeasts in feces collected in a serial way for a parasitological study was reported. A 49- year-old VIH positive man was admitted at ward on account of fever, dysuria and back pain. Criptococal meningitis diagnosis was made and speci1c treatment was started. Pneumocystis jiroveci pneumonia could not be assessed. Bacteriological culture of urine samples was negative. A parasitological study of feces, performed on account of mild diarrhoea, was negative for enteroparasites. Microscopy of concentrated fresh sample revealed rounded thick wall yeasts surrounded by a clear halo, resulting from a sliding movement of the feces around it, consistent with Cryptococcus neoformans. Direct examination with optic microscope of a sputum sample showed capsulated yeasts which developed C. neoformans in culture. These findings are in accordance with respiratory secretions swallowed by the patient, who carried a pulmonary localization of this disseminated mycosis.
