Resumen

Objective: To show the value of special MRI techniques (spectroscopy, diffusion, perfusion, ADC, tractography and functional MRI (fMR) for: 1. To evaluate the intrinsic characteristics ( biochemical, grade of malignancy and infiltration) and extrinsic (relationship with the regular brain structures from the expansive processes of the central nervous system). 2. To decide with the fMR the appropriate strategy and surgical approach for each case. Material and method: We present two 35 and 40 year old male patients, with tumoral injuries in eloquent motor areas from the right cerebral hemisphere. The surgical planning and resection was based on the fMR research (Philips 1.5 T Intera Achieva). Results: The location of the eloquent areas through fMR and the findings of the imagenologics techniques described, contributed to determine the strategy and planning the surgical approach, obtaining an excellent correlation between the imagenologics and operative findings. Conclusion: In brain tumors located in eloquent areas the RMI investigation with special techniques is of main importance for: 1. The probable diagnosis of kind of tumor, grade of malignancy and infiltration. 2. The planning of the surgical strategy, to reduce the postoperative neurologic deficit.
