Objective. To describe and analyze a series of patients in whom neuroendoscopy guided by neuronavigation was used to resolve different neurosurgical pathologies. Material and method. A retrospective analysis was made of the charts of patients in whom a combination of the neuroendoscope with a rigid rod lens of 0° or 30° STORZ-DECQ and a neuronavigator (BRAIN LAB Vectorvisión II ó ELECTA) was used. Results. Since April 2001 until December 2006, we have operated on 14 patients combining both techniques. The ages of the patients ranged from 10 months to 10 years (mean 7,5 years). Surgeries were performed because of arachnoid cysts near or within small ventricles in six cases, hypothalamic tumors in four, isolated ventricles in three, and a porencephalic cavity in one case. The accuracy of the neuronavigational procedure was always less than 1mm, however, the ventricular system could not be accessed in one of the 14 patients. In the remaining 13 patients, surgical planning was achieved adequately . Conclusion. Neuroendoscopy guided by frameless stereotaxic navigation is an accurate and safe technique in pathologies that can be solved endoscopically and when the ventricles are small.
