Spine surgery education has two steps: formation and perfectioning. Formation starts with the residency and is completed with formation courses, were knowledges, attitudes and basic dexterities are acquired. Perfectioning develops during all the active professional life through fellowships and courses. Fellowships have an extension of one or two years and can be general or specialized in one pathology. In them, basic knowledges and the performance of diagnostic and therapeutic abilities are deepened, training in basic investigation and expertise in complex surgeries are encouraged. Courses have a one or two days extension, allow constant improvement and are referred to one issue or technique. In both, residencies and fellowships, is necessary to acquire confidence in order to know how to communicate with the patients and their families and, how to interact with the industry. Ethics and professionalism are molded during these two steps. For this is necessary to count with mentors capable of understanding this process of formation and perfectioning, in order to prevail with their guide and advice. All this process, inevitably leads us to ask: Is it necessary a certification in "spine surgery" as a new specialty? Spine surgery has a distinct organ system and knowledge body, a huge number of physicians devoted to it and societies with appropriate specialty training programs. There are contrary opinions about this process and the issue has not been settled yet.
