Objective. In the resolution of the aneurismatic pathology the election an adequate surgical strategy is still important, especially on those with difficult access due to the anatomical location; this is the case of the aneurisms of the segment P2. The objectives of this work are: 1. To know the different options so as to treat this pathology surgically. 2. To highlight the difficulties and cares that should be taken into account at the moment of treating this kind of aneurism. Clinical presentation. A case of a 48 year-old female patient is presented. The patient was admitted with migraine, vomit, nape rigidity, and diplopia (Hunt & Hess II). Admission CT shows subaracnoid hemorrhage (Fisher III). Cerebral angiography evidences an aneurism of the segment P2 of the Posterior Cerebral Artery. Intervention The patient is surgically intervened through a subtemporal approach and g the aneurism is clipped without compromising the distal segment of the artery. Conclusion. The aneurisms of the P2 segment are very infrequent and offer great technical difficulties to be treated due to their location in the deep parts of the temporal lobe in relation to the peritroncals cisterns. All the measures that contribute to a lower cerebral retraction constitute, with the meticulous anatomic knowledge of this zone, the base for its treatment, obtaining the best results.
