Objective. To pesente a Primary Angiitis of the CNS is an uncommon entity of truly varied presentation. Description. We present a 28 year old patient with a history of episodes of aphasia and right hemiparesis with images suspicious of brain tumor. Intervention. New studies made us question the diagnosis (fIRM, spectroscopy and angiography). Systemic angiitis and infectious disease was rejected. Open biopsy was done with an unclear result. We assume a conservative treatment. At five months patient present intracranial hypertension symptoms and images showed a frontotemporal quistic lesion. A new surgery was necessary. Histopathology diagnosis was cerebral vasculitis. He started treatment with corticoids and ciclofosfamida showing a good response without relapses at 8 months of follow-up. Then we make a review of the literature.
