Abstract

Metabolic syndrome (MS) is a cluster of metabolic abnormalities involving an increased long-term risk of cardiovascular disease and diabetes. MS characteristics in patients with acute coronary syndromes (ACS) are almost unknown. Objective To assess the prevalence, characteristics and prognostic value of MS in ACS. To evaluate its correlation with coronary risk factors and dietary habits, and to analyze a recently proposed new definition. Research Design and Methods Consecutive patients admitted to a Coronary Care Unit with acute myocardial infarction or unstable angina were included. The diagnosis of MS followed NCEP-ATP III criteria. The new definition proposed by the International Diabetes Federation was also examined. Results Two hundred and thirty nine patients were included in the study. MS was present in 53.3% and showed higher frequency in women (OR 2.53; 95% CI: 1.24-5.18) (p = 0.005). It did not show a significant relation either to in-hospital prognosis or dietary habits. Prevalence was almost identical (53%) according to the new definition and although it had significant prognostic value in the univariate analysis, significance was lost in the multivariate analysis. Conclusions The prevalence of MS in ACS is higher than that reported in other cohorts. It does not seem to have short-term prognostic value. The new definition did not show a different prevalence and did not add independent prognostic information.
