Abstract

The prevalence of heart failure and diabetes is in constant increase. Both are strongly associated. Diabetes is a strong predictor of heart failure, due to the coronary artery disease and metabolic disorders related to insulin resistance that cause contractile dysfunction. Patients with heart failure and diabetes have a poor evolution. Vasodilator betablockers and angiotensin converting enzyme inhibitors exert favorable influence; the use of glitazones is still controversial. Heart failure can indeed generate diabetes, because of sympathetic nervous system activation and medical treatment side effects. An adequate understanding of these facts is fundamental to make correct decisions and improve the outcome of patients.
