In patients with aneurysmal dilatation of the aortic root and severe valve insufficiency with a structurally normal aortic valve, it is feasible to perform an aortic valve-sparing operation to correct the valve regurgitation, without the need for anticoagulation. We present the mid-term (3 years) postoperative outcome of a 71 year-old patient with a diagnosis of aortic root aneurysm and severe aortic insufficiency, who underwent surgery of the ascending aorta sparing the aortic valve. The Doppler-echocardiogram performed three years after surgery showed a decrease in LV systolic and diastolic dimensions with a peak systolic aortic flow velocity of 1.1 m/ sec and a peak gradient of 35 mmHg, and without valve regurgitation. The aortic root valve-sparing operation exhibited a reliable durability during a mid-term follow up.
