Abstract

Introduction Left ventricle (LV) severe diastolic dysfunction and particularly echocardiography indexes of elevated LV diastolic pressures are indicators of worse prognosis after an acute myocardial infarction (AMI). In order to accurately identify patients with increased ventricular filling pressures, we attempted to combine mitral flow parameters with other diastolic function parameters. Objective To assess the diastolic function (DF) in acute myocardial infarction (AMI) and its relation with the presence of events during long term follow up. Material and methods Twenty five patients were prospectively assessed with Doppler- echo within 72 hours post admission between June 2002 and February 2004 (mean age 59±2 years). Ejection fraction (Fey) was estimated from the 4C and 2C apical views. The DF was assessed by mitral flow, pulmonary vein flow, colour M-mode mitral flow and tissue Doppler (TD) mitral annulus. According to the presence of events throughout follow up (angina, heart failure, malignant arrhythmias), patients were divided in two groups: with events (G1) (8 patients [32%]) and without events (G2) (17 patients [68%]) with an average follow up period of 16±5 months. Results No significant differences were found between both groups after the assessments of Fey and DF derived from mitral flow, pulmonary vein flow, colour M-mode mitral flow and tissue Doppler. However, the combination of mitral flow peak E-wave velocity ratio to colour M-mode mitral flow propagation velocity (E/Vp) (G1 3.53±0.6 versus G2 2.15±0.2 [p=0.02]) and mitral flow peak E-wave velocity ratio to tissue Doppler peak mitral annular velocity at the septum level (Eas) (E/Eas) (G1 10.7±0.7 versus G2 6.9 ± 0.6 [p=0.004]) showed significant differences. Conclusion In the evaluation of the DF in patients with AMI assessed within 72 hours, E/Vp and E/Eas ratios showed significant differences between patients with and without events during long term follow up. These findings suggest the influence of load conditions in the DF during AMI and the need to normalize the parameters assessed through preload, using the mitral flow peak E-wave velocity.
