Pure cocaine and contaminated cocaine with levamisole produce multiple cutaneous and systemic manifestations, being midline destructive lesions and ANCA-like vasculitis the most frequent ones. The pathophysiological bases of these manifestations are based on secondary vascular damage to vasculitis and/or vasculopathy caused by both substances and in many cases it may be found an overlapping of the clinical forms, which might lead to erroneous diagnoses.
The patterns of positivity of the autoantibodies, the pattern of skin lesions and the social context of the patient will be key in the proper approach and management of these patients.
The following case describes a patient with cutaneous, nasal and pulmonary manifestations due to cocaine / levamisole consumption. Also, a review of the literature is carried out.
