Background. The Metabolic Syndrome (MS) increases the cardiometabolic risk, and the prevalence is approximately 20%. Argentine's data are poor. Objective. 1) Find the prevalence of Metabolic Syndrome according to the different definitions among the population that visits the cardiologist. 2) Find the prevalence of the different traits of the Metabolic Syndrome among this population. 3) Establish the correlation between perceive clinical diagnosis by the cardiologists and the presence of Metabolic Syndrome traits according to the Adult Treatment Panel III (ATP) III and the International Diabetes Federation (IDF). 4) Establish relationship between perceived cardiovascular risk and Framingham score. Participants and methods. Design: multicentric, observational and cross-sectional study. Investigators: cardiologist. Members of the Cardiology Society of the country. Inclusion: participants ≥21 years old, both sex and first visit at cardiology practice office. Exclusion: Diabetes mellitus diagnosis American Diabetes Association (ADA) criterion). A case report form was completed with: antecedent, reason of consult, arterial blood pressure, anthropometric and biochemical result (glucose, total cholesterol -TC-, high density lipoprotein-cholesterol -HDL-C- and triglycerides -TG-). The processing data was centralized at Epidemiology and Cardiovascular Prevention Committee of the Buenos Aires Society of Cardiology. Results. On 339 participants the prevalence of the MS for ATP-III was 48.9% and 60.1% for IDF. The age was 58.3±1.0 years and 49.3% were female. The prevalence of the diagnosis criteria was: hypertension 93.3% (SBP 147.5±19.4/DBP 88.9±14.1 mm Hg) abdominal circumference 72.8% (113.7±8.5 cm), hypertriglyceridemia 80.1% (236.7±19.1 mg/dl), low HDL-C 86.7% (37.0±1.4 mg/dl) and hyperglycemia 38.5% (102±37.5 mg/dl). The relationship between "perceive clinical diagnosis" and ATP-III diagnosis criterion was moderate (Kappa index 0.53) and good with IDF criterion (Kappa index 0.74). The relationship between "cardiovascular risk perceive" vs Framingham score was null (Kappa index 0.033). Conclusion. 1) The prevalence of the MS in cardiology patients is twice as much as the general population is: 48.9% (ATP-III) and 60.1% (IDF). The triad hypertension, low HDL-C and hypertriglyceridemia was the most prevalent, 2) the relationship between "perceive clinical diagnosis" of MS and IDF criterion was good The relationship between "cardiovascular risk perceive" and Framingham score was null.
