Introduction. To investigate neurohumoral activation in patients with myocardial injury but no heart failure clinical syndrome. Objectives. To determinate neurohumoral activation, brain natriuretic peptide (BNP), in patients with risk factor for heart falure and patients with left ventricular dysfunction with o without clinical heart failure. Methods and material. Patients at emergency of east general hospital "Dr. Domingo Luciani" in Caracas, Venezuela. Test Elecsys pro BNP was taken in 42 patients with systemic hypertension, coronary artery disease o diabetes mellitus with o without clinical syndrome. another group, 39 health persons. Results. The mean age was 63.02 years; hypertension was present in 81% of patients; BNP was high in 59,5% (mean 5.720 pg/ml). mean level in healthy group (61.11 pg/ml). BNP was higher in functional class III, low ejection fraction, left ventricular hypertrophy or heart rate >80 beats per minute, 65 years old or more. Conclusions. Brain natriuretic peptide is related with functional class, left ventricular dysfunction, venocapilar hypertension, left ventricular hypertrophy, heart rate >80 bpm.
