Many patients who survive an acute myocardial infarction (MI) remain at risk of sudden cardiac death despite optimal medical treatment. Identifying patients at risk of sudden cardiac death and selection for implantation of an implantable cardioverter defibrillator is made on the basis of the value of the ejection fraction left ventricular. There are other tests that help identify patients at risk for sudden cardiac death. Among them are methods based on the ECG (signal-averaged and T-wave alternans), the Holter (heart rate variability and turbulence), images (cardiac magnetic resonance). More recent evidence based on other indices of 12-lead ECG and genetic tests for risk prediction. We will review the current evidence for the use of these tests in survivors of acute myocardial infarction, and analize their profits to be used as guidelines for preventing sudden cardiac death and identify possible candidates for an implantable cardioverter defibrillator.
