Undoubtedly the PARADIGM HF study is fundamental in the treatment of heart failure by demonstrating a clinically significant benefit in terms of overall mortality reduction, at the expense of cardiovascular death with a great impact also on sudden death. The present paper discusses its importance, the weight of its evidence (which in this group of patients is definitive) and different clinical situations in which it is still debated about its indication: The stable patient, the patient with subtherapeutic doses, their use in patients who did not previously receive enalapril and the benefit in subtherapeutic doses. The weight of evidence is inconceivable; the same clinical practice will consolidate its indication. The critical analysis of the available data allows a positioning before different clinical situations.
