Background: Several controversies still remain with regard to the pin placement configuration in supracondylar humeral fractures in children. A recently described lateral cross-wiring technique would be biomechanically better, avoiding ulnar nerve injury. To our knowledge there are no studies comparing this technique with other wiring configurations. The aim of this study is to compare cosmetic and functional results of two lateral-entry wiring techniques. Methods: 104 patients, 62 with cross configuration (average age 5.9 ± 1.8 years) and 42 with divergent lateralentry configuration (average age 7.4 ± 0.9 years). The average follow-up was 14.2 ± 1.0 months in group A and 36.2 ± 1.6 months in group B. We evaluated the functional results and cosmesis using Flynn's criteria. We analyzed further complications and postoperative loss of reduction. Results: The functional and cosmetic results did not differ significantly between groups. The complication rate was slightly higher in group A (11.4% vs. 9.4%), although excessive granulation tissue around the pins was the only parameter with a statistically significant difference. Discussion: Although the cross-wiring configuration would theoretically have greater rotational stability, these biomechanical advantages are not reflected in the functional and cosmetic outcomes.
