ABSTRACT Background: In late years two techniques have been published to measure acetabular anteversion using simple radiographs. To our knowledge, no reports have compared their accuracy. The goal of this study was to determine these techniques' accuracy and reliability using simple radiographs of an in-vitro model. Methods: An in-vitro model was used based on a plastic pelvis from which x-rays were taken in 5 different anteversion positions with the same inclination angle. The methods described by Pradhan and Widmer were evaluated. For each different orientation, radiological anteversion was measured with both techniques by 10 observers. We assessed inter-observer reliability, methods' agreement, and measurements' accuracy. Results: The results showed that both methods have high inter-observer reproducibility. Using the Widmer method, the measurements obtained were systematically higher than the real size. As regards the comparison with the gold standard, the Pradhan technique was more accurate. Conclusions: Both techniques exhibited a tendency to larger measurements than real ones. Although the correlation between methods is acceptable, 0.96 (p < 0.01),the Widmer technique is less reliable. The Pradhan method is more reliable to assess acetabular malpositioning, although for component migration, neither Pradhan nor Widmer techniques are useful. Planar anteversion should be just part of the information required for decision making.
