Abstract

Background: To evaluate the mid-terms results in hip revisions, both septic and aseptic, using uncemented cups. Methods: Between 1990 and 2006 we used non cemented acetabular components for hip revision in 208 cases, 201 patients, could be evaluated. 157 cases of 153 patients, with a 7 and 8 years average check. In our series, 91 were female and 66 male, the average age was 66, with a range between 39 and 83 years. The base patology was, mechanical in 93 cases, enfeccious in 31 cases, osteolisis in 11 cases, breaking implant (linnercotilo) 10 cases, prostetic dislocation in 12 cases. Results: It took in account the sieze of the used acetabulum, 44,5% of cases were of diameter bigger than 60 mm. 48,5% between 54 and 60 mm. 7% less than 54 mm. We seek the relation betweeen the vertical desplazement and horizontal with the diameter of the used used acetabular component, the componets smaller than 60mm. Have better tendencies to be ascended implanted (19,7%) compeared with the teardrop line immage. While the intra pelvis protrusio acetabuli with protution medially of the acetabulum looks bigger in bigger acetabulum (32,8%) Bone fixation was acomplished in 148 cases, presenting fibrous fixation in 4 cases, which did not present simptoms nor surgical need. Working loose the acetabular component, two of them of mechanical cause and tree infeccious. The components that are asociated to bone grafts have a 92,6% chances to sourvive while in the ones which was not used the bone grafts was of 100%. In our series we had 3 cases of prostetic luxation, two of these with surgical treatement. Conclusions: The hip revision with uncemented cups has a failure rate of 0.5% in the midd-term. We consider it a desirable alternative, after examining each individual case.
