Background: onychocryptosis is commonly seen in our specialty. Many treatments have been described in the literature with varying results. The purpose of this study is to compare the clinical-functional results of patients treated for onychocryptosis with two different techniques: ungueal matrix preservation, and ungueal matrix excision. Methods: Between March 2002 and October 2006 we evaluated 2 groups of patients surgically treated for onychocryptosis: Group A: without matrix excision (54 cases); Group B: with matrix excision (33 cases). In both groups the average age was 35. We analyzed the clinical evolution, cosmetic and footwear comfort, and complications and recurrences. Average follow-up: 19 months in Group A and 24 months in Group B. The statistical method was used to compare both groups. Results: Recurrence: Group A: 11%, and Group B: 6%. POP pain: based on the visual scale both groups presented less than 0.5/10. In Group A, 16.6% had mild pain, and in Group B 3%. Cosmesis: 100% satisfaction in both groups. Conclusions: Onychectomy treatment without matrix excision is indicated in primary cases. We indicate surgery with partial excision of the matrix (involved area) in relapses, since recurrence rates are lower.
