Fat Embolism Syndrome (FES) is an uncommon entity characterized by a difficult clinical diagnosis and high morbi-mortality in polytrauma patients. It is the clinical manifestation of the presence of fatty cells in the systemic circulation and it is mainly characterized by the development of respiratory failure, neurologic symptoms and petechial rash. Although it has been largely associated with long-bone fractures, it may be caused by other types of pathologies. Clinical findings have been classically used for the diagnosis. Therefore, we report on four cases where brain Magnetic Resonance Imaging (MRI) had a key role in the diagnosis.
