Objectives. To review the clinical, sonographic, mammographic, pathological and magnetic resonance imaging find-ings of cellular fibroadenomas and discuss the differential diagnosis of other breast masses. Materials and Methods. Medical record reports from 22 patients admitted to Hospital Italiano de Buenos Aires with histopathologically confirmed diagnosis of cellular fibroadenoma between 2001 and 2012 were reviewed. Clinical, sonographic, mammographic, pathological and magnetic resonance imaging findings were analyzed. Results. All patients (n = 22 females) with a pathology diagnosis of cellular fibroadenomas were evaluated. The average age was 23 years (range 11-46 years old). 20 patients had clinically solitary palpable lumps of rapid growth, in 20 cases and multiple lesions in 2 cases. On mammography (performed in 2/22 patients), these lesions appeared as homogeneous high-density masses with smooth borders. Sonograms (22/22 patients) showed well-circum-scribed, ovoid, lobulated (14 cases) and round (8 cases) hypoechoic masses. MRI findings (3/22 patients) revealed round masses with intense enhancement of the tumors except for septations. Needle core biopsy performed in 4 cases showed suggestive changes of cellular fibroadenoma. Surgical excision was indicated in all cases. Macroscopically, cellular fibroadenomas ranged from 2, 3 to 14cm (mean 6 cm).Microscopically, all lesions consisted of biphasic neoplasms with stromal hypercellularity and foci of epithelial hyperplasia. The differential diagnoses considered were phyllodes tumor, typical fibroadenomas and juvenile hypertrophy. Conclusions. The main features of cellular fibroadenoma on ultrasound are well-circumscribed, ovoid, lobulated, hypoe-choic masses, with heterogeneous echotexture. Clinical, sonographic and radiological findings of cellular fibroadenomas overlap with classic fibroadenoma and phyllodes tumor, and their differentiation is assumed from rapid growth and stromal hypercellularity.
