Abstract

A 64-year old man came to consultation with painful ulcerations in distal lower limbs, that appear in summer that lasted for 10 months. He was a fatty man, with arterial hypertension and non-dependent insulin diabetes. He also suffered of paraesthesias since 20 months ago. Electromiography showed the presence of mononeuritis multiplex that evolved into a mixed axonal polineuropathy. Skin necrosis associated with fibrinoid trombii and deposits of hyaline material with mixed inflammatory infiltrate was seen in skin biopsy. A diagnosis of livedoid vasculitis was made. This is not a true vasculitis, but an occlusive phenomenon that should be differentiated from other vasculitis (cutaneous small vessels vasculitis, cutaneous polyarteritis nodosa, chronic venous stasis, pyoderma gangrenosum, and others). Neurological involvement is exceptional in this vasculopathy, having been described only 5 cases with this association
