RESUMEN

Introducción. La prevalencia de anemia por déficit de hierro (ADH) es alta en niños menores de dos años, especialmente en poblaciones carenciadas.
Objetivo. Estudiar algunos determinantes sociales y biológicos que se asocian con la ADH en niños de 12 a 23,9 meses de edad de la región del Noreste Argentino durante los años 2004-2005.
Metodología. Diseño transversal. Las variables explicativas fueron organizadas en tres niveles: distal (asistencia alimentaria, necesidades básicas insatisfechas, cobertura médica del jefe de hogar), intermedio (lactancia materna, suplementación con hierro y consumo de hierro) y proximal (estado nutricional, edad gestacional, peso al nacer, edad y sexo). La asociación entre las variables seleccionadas y ADH (Hb < 11 g/dl, ferritina < 12 ng/dl, glóbulos blancos < 15 000/ml) fue examinada con modelos de regresión logística con selección jerárquica de las variables.
Resultados. La falta de asistencia alimentaria [OR 1,85 (1,14; 3,02)] y el consumo inadecuado de hierro [OR 2,60 (1,18; 5,71)] fueron asociados a ADH. Cuanto menor la edad gestacional [OR 0,89 (0,81; 0,97)] y menor la edad en meses [OR 0,90 (0,84; 0,96)], mayor la prevalencia de anemia.
Conclusiones. Este estudio pone en evidencia la estrecha y compleja relación entre los determinantes sociales y una enfermedad altamente prevalente en niños pequeños, como la anemia. Los programas de asistencia alimentaria tendrían un efecto protector sobre la ADH; el consumo de alimentos fortificados mejoraría la ingesta de hierro en los niños menores de dos años de edad. Se resalta, además, la necesidad de realizar evaluaciones del impacto de las políticas sobre la salud de la población.

ABSTRACT

Introduction. The prevalence of iron deficiency anemia (IDA) is high among infants younger than two years old, especially in disadvantaged populations.
Objective. To study certain social and biological determinants associated with IDA in children aged 12 to 23.9 months old in Northeast Argentina in the period 2004-2005.
Methodology. Cross-sectional design. Explanatory outcome measures were organized in three levels: distal (food assistance, unmet basic needs, and head of household with medical coverage), intermediate (breastfeeding, iron supplementation, and iron intake), and proximal (nutritional status, gestational age, birth weight, age and sex). The association between selected outcome measures and IDA (Hb < 11 g/dl, ferritin < 12 ng/dl, WBCs < 15 000/ml) was assessed using logistic regression models with hierarchical selection of outcome measures. Results. Lack of food assistance (OR: 1.85 [1.14, 3.02]) and inadequate iron intake (OR: 2.60 [1.18, 5.71]) were associated with IDA. The prevalence of anemia was higher with a younger gestational age (OR: 0.89 [0.81, 0.97]) and a younger age in months old (OR: 0.90 [0.84, 0.96]).
Conclusions. This study evidences the strong and complex relationship between social determinants and anemia, a disease that is highly prevalent among young children. Food assistance programs may have a protective effect against IDA; consumption of fortified foods may improve iron intake in infants younger than two years old. In addition, emphasis is placed on the need to assess the impact of policies on population health.

Concentración de insulina e índices de insulinosensibilidad en niños y adolescentes sanos

Insulin level and insulin sensitivity indices among healthy children and adolescents

RESUMEN

Introducción. Existe escasa información acerca de los valores de referencia de la insulina y de los índices de insulinosensibilidad en pediatría.
Objetivo. Describir la variación de insulina e índices subrogantes de insulinosensibilidad en la etapa pediátrica.
Población y métodos. Variación de la concentración de insulina en ayuno y de los índices subrogantes, como el modelo de evaluación homeostática de resistencia a la insulina (homeostasis model assessment of insulin resistance; HOMA-IR, por sus siglas en inglés), en niños sanos con la edad, el índice de masa corporal, estadio puberal (EP), la concentración de IGF-I, colesterol total y triglicéridos.
Resultados. Se incluyeron 226 niños sanos (1-18 años). La insulina aumentó con la edad, el índice de masa corporal, el EP, los niveles de IGF-I y triglicéridos (r = 0,38; p < 0,0001). Los niños prepuberales > 7,5 años presentaron mayores valores de insulina [mediana (Pc3 y Pc97) ^UI/ml: 5,0 (1,7-9,6)] que los prepuberales < 7,5 años [2,9 ^UI/ml (1,3-10,9); p < 0,01]. En la pubertad (del EP II al EP V), la insulina fue mayor en las niñas que en los varones [(7,4 (1,8-16,9) versus 5,8 (1,8-12,9); p < 0,01]. El índice HOMA-IR aumentó en el grupo prepuberal > 7,5 años: 1,1 (0,3-2,0) versus niños < 7,5 años: 0,6 (0,3-1,4; p < 0,01). Los grupos puberales presentaron niveles más elevados de insulina y de HOMA-IR respecto de los niños prepuberales (p < 0,001).
Conclusiones. La insulina y el índice HOMA-IR mostraron los cambios fisiológicos conocidos en niños y adolescentes. Valores de insulinemia en ayuno de 10 juUI/ml en prepúberes y 17 juUI/ml y 13 juUI/ml en niñas y niños púberes respectivamente pueden ser considerados como valor límite aceptable en niños sanos. HOMA-IR > 2,0 y > 2,6 en prepúberes y púberes, respectivamente, podrían alertar a los pediatras sobre un posible estado de insulinorresistencia.

ABSTRACT

Introduction. Information on insulin reference values and insulin sensitivity indices in the field of pediatrics is scarce.
Objective. To describe insulin range and insulin sensitivity surrogate indices during childhood.
Population and methods. Fasting insulin level range and surrogate indices, such as the homeostasis model assessment of insulin resistance (HOMA-IR), among healthy children and adolescents by age, body mass index, pubertal stage (PS), insulin-like growth factor-1 (IGF-1), total cholesterol, and triglycerides.
Results. Two hundred and twenty-six healthy children and adolescents (1-18 years old) were included. Insulin increased with age, body mass index, pubertal stage, IGF-1 and triglyceride levels (r2= 0.38, p < 0.0001). Prepubertal children > 7.5 years old had higher insulin levels [median (P3 and P97) pIU/ml: 5.0 (1.7-9.6)] than prepubertal children < 7.5 years old [2.9 pIU/ml (1.3-10.9), p < 0.01]. During puberty (from PS II to PS V), insulin was higher in girls than in boys [7.4 (1.8-16.9) versus 5.8 (1.8-12.9), p < 0.01]. The HOMA-IR index increased in the group of prepubertal children > 7.5 years old: 1.1 (0.3-2.0) versus children < 7.5 years old: 0.6 (0.3-1.4, p < 0.01). The insulin level and HOMA-IR results were higher in pubertal children compared to the prepubertal group (p < 0.001).
Conclusions. Known physiological changes were observed in both insulin levels and the HOMA-IR index among children and adolescents. A fasting blood insulin level of 10 pIU/mL in prepubertal children and of 17 pIU/ml and 13 pIU/ml in pubertal girls and boys, respectively, may be considered as an acceptable cut-off value in healthy children. A HOMA-IR value > 2.0 and > 2.6 in prepubertal and pubertal children, respectively, may be considered a warning sign for pediatricians to further investigate insulin resistance.

Clinical and laboratory signs associated to serious dengue disease in hospitalized children

ABSTRACT

Objective. To evaluate the validity of clinical and laboratory signs to serious dengue disease in hospitalized children.
Methods. Retrospective cohort of children (<18 years) hospitalized with dengue diagnosis (2007-2008). Serious dengue disease was defined as death or use of advanced life support therapy. Accuracy measures and area under the receiver operating characteristic curve were calculated.
Results. Of the total (n= 145), 53.1% were female, 69% aged 2-11 years, and 15.9% evolved to the worse outcome. Lethargy had the best accuracy (positive likelihood ratio >19 and negative likelihood ratio <0.6). Pleural effusion and abdominal distension had higher sensitivity (82.6%). History of bleeding (epistaxis, gingival or gastrointestinal bleeding) and severe hemorrhage (pulmonary or gastrointestinal bleeding) in physical examination were more frequent in serious dengue disease (p < 0.01), but with poor accuracy (positive likelihood ratio = 1.89 and 3.89; negative likelihood ratio = 0.53 and 0.60, respectively). Serum albumin was lower in serious dengue forms (p < 0.01). Despite statistical significance (p < 0.05), both groups presented thrombocytopenia. Platelets count, hematocrit, and hemoglobin parameters had area under the curve <0.5.
Conclusions. Lethargy, abdominal distension, pleural effusion, and hypoalbuminemia were the best clinical and laboratorial markers of serious dengue disease in hospitalized children, while bleeding, severe hemorrhage, hemoconcentration and thrombocytopenia did not reach adequate diagnostic accuracy. In pediatric referral hospitals, the absence of hemoconcentration does not imply absence of plasma leakage, particularly in children with previous fluid replacement. These findings may contribute to the clinical management of dengue in children at referral hospitals.

Costs of hospitalization in preterm infants: impact of antenatal steroid therapy

ABSTRACT

Objective. To estimate the costs of hospitalization in premature infants exposed or not to antenatal corticosteroids (ACS).
Method. Retrospective cohort analysis of premature infants with gestational age of 26 -32 weeks without congenital malformations, born between January of 2006 and December of 2009 in a tertiary, public university hospital. Maternal and neonatal demographic data, neonatal morbidities, and hospital inpatient services during the hospitalization were collected. The costs were analyzed using the microcosting technique.
Results. Of 220 patients that met the inclusion criteria, 211 (96%) charts were reviewed: 170 newborns received at least one dose of antenatal corticosteroid and 41 did not receive the antenatal medication. There was a 14-37% reduction of the different cost components in infants exposed to ACS when the entire population was analyzed, without statistical significance.
Regarding premature infants who were discharged alive, there was a 24-47% reduction of the components of the hospital services costs for the ACS group, with a significant decrease in the length of stay in the neonatal intensive care unit (NICU). In very-low birth weight infants, considering only the survivors, ACS promoted a 30-50% reduction of all elements of the costs, with a 36% decrease in the total cost (p= 0.008). The survivors with gestational age <30 weeks showed a decrease in the total cost of 38% (p= 0.008) and a 49% reduction of NICU length of stay (p= 0.011).
Conclusion. ACS reduces the costs of hospitalization of premature infants who are discharged alive, especially those with very low birth weight and <30 weeks of gestational age. Key words: preterm infant; costs and cost analysis; hospitalization; glucocorticoids.

RESUMEN

El óxido nítrico inhalatorio (ONi) es actualmente la terapia de primera línea en la insuficiencia respiratoria hipoxémica grave del recién nacido; la mayor parte de los centros neonatales de regiones en Chile no cuentan con esta alternativa terapéutica.
Objetivo. Determinar el costo-efectividad del ONi en el tratamiento de la insuficiencia respiratoria asociada a hipertensión pulmonar del recién nacido, comparado con el cuidado habitual y el traslado a un centro de mayor complejidad.
Pacientes y método. Se modeló un árbol de decisiones clínicas desde la perspectiva del sistema de salud público chileno; se calcularon razones de costo-efectividad incremental (ICER), se realizó análisis de sensibilidad determinístico y probabilístico, se estimó el impacto presupuestario, software: TreeAge Health Care Pro 2014.
Resultados. La alternativa ONi produce un aumento promedio en los costos de 11,7 millones de pesos por paciente tratado, con una razón de costo-efectividad incremental comparado con el cuidado habitual de 23 millones de pesos por muerte o caso de oxigenación extracorpórea evitada. Al sensibilizar los resultados por incidencia, encontramos que a partir de 7 casos tratados al año resulta menos costoso el óxido nítrico que el traslado a un centro de mayor complejidad.
Conclusiones. Desde la perspectiva de un hospital regional chileno incorporar ONi en el manejo de la insuficiencia respiratoria neonatal resulta la alternativa óptima en la mayoría de los escenarios posibles.

ABSTRACT

Inhaled nitric oxide (iNO) is currently the first-line therapy in severe hypoxaemic respiratory failure of the newborn. Most of regional neonatal centres in Chile do not have this therapeutic alternative.
Objective. To determine the cost effectiveness of inhaled nitric oxide in the treatment of respiratory failure associated with pulmonary hypertension of the newborn compared to the usual care, including the transfer to a more complex unit.
Patients and method. A clinical decision tree was designed from the perspective of Chilean Public Health Service. Incremental cost effectiveness rates (ICER) were calculated, deterministic sensitivity analysis was performed, and probabilistic budget impact was estimated using: TreeAge Pro Healthcare 2014 software.
Results. The iNO option leads to an increase in mean cost of $ 11.7 million Chilean pesos (D 15,000) per patient treated, with an ICER compared with the usual care of $ 23 million pesos (D 30,000) in case of death or ECMO avoided. By sensitising the results by incidence, it was found that from 7 cases and upwards treated annually, inhaled nitric oxide is less costly than the transfer to a more complex unit.
Conclusions. From the perspective of a Chilean regional hospital, incorporating inhaled nitric oxide into the management of neonatal respiratory failure is the optimal alternative in most scenarios.

Contaminación intradomiciliaria por material particulado (MP2,5) en hogares de recién nacidos

Indoor air pollution by fine particulate matter in the homes of newborns

RESUMEN

Introducción. La contaminación de aire por material particulado (MP) es un importante problema de salud pública. En Chile se ha estudiado la contaminación atmosférica y MP^ y escasamente aire interior y MP2^. Como los recién nacidos y los lactantes pasan la mayoría del tiempo en el hogar, es necesario evaluar la exposición a la contaminación intradomiciliaria en esta población susceptible.
Objetivo. Determinar la concentración de MP25 en hogares de recién nacidos e identificar las fuentes de emisión contaminante.
Pacientes y método: en 207 hogares se recolectó la concentración de MP25 ([MP25]) por 24 h y se evaluó la información sociodemográfica y los factores ambientales (calefacción, ventilación, tabaquismo intradomiciliario y aseo del hogar).
Resultados: la [MP2J mediana fue 107,5 μg/m3. El antecedente «asma familiar» se asoció a menor [MP25] (p= 0,0495). Hogares sin uso de calefacción mostraron la menor [MP2,5] mediana, 58,6 μg/m3, mientras los que usaron leña, parafina, electricidad entre 112,5 y 114,9 μg/m3 y brasero 162,9 μg/m3. Hogares que usaron leña tuvieron diferencias significativas en [MPJ mediana (p = 0,0164) al usar estufa de combustión completa (98,2 μg/m3) versus cocina a leña (112,6 μg/m3) y salamandra (140,6 μg/m3). En el 8,7% de los hogares se reportó consumo de cigarrillos, no asociándose a [MP2 5]. La ventilación se asoció a mayor [MP25] mediana (120,6 vs. 99,1 μg/m3; p = 0,0039).
Conclusión. Hubo hogares con [MP2,5] elevada, siendo el consumo residencial de leña casi universal y asociado a [MP25]. La ventilación natural incrementó el MP25 interior, probablemente por infiltración desde el exterior.

ABSTRACT

Introduction. Air pollution by particulate matter (PM) is a major public health problem. In Chile, the study has focused on outdoor air and PM^ rather than indoor air and PM25. Because newborns and infants spend most of their time at home, it is necessary to evaluate the exposure to indoor air pollution in this susceptible population.
Objective. To determine concentration of PM25 in the homes of newborns and identify the emission sources of the pollutants. Patients and method: The PM^ concentration ([PM25]) was collected over a 24 hour period in 207 households. Baseline sociodemographic information and environmental factors (heating, ventilation, smoking and house cleaning), were collected.
Results. The median [PM25] was 107.5 μg/m3. Family history of asthma was associated with lower [PM2.5] (P = .0495). Homes without heating showed a lower median [PM25], 58.6 μg/m3, while those using firewood, kerosene, and electricity ranged between 112.5 and 114.9, and coal users' homes reached 162.9 μg/m3. Wood using homes had significant differences (P=.0164) in median [PM2.5] whether the stove had complete combustion (98.2 μg/m3) vs. incomplete (112.6 μg/m3), or a salamander stove (140.6 μg/m3). Cigarette smoking was reported in 8.7% of the households, but was not associated with the [PM25]. Ventilation was associated with a higher median [PM2.5] (120.6 vs. 99.1 μg/m3, P= .0039).
Conclusion. We found homes with high [PM25]. Residential wood consumption was almost universal, and it is associated with the [PM25]. Natural ventilation increased MP^, probably due to infiltration from outside.

Periviabilidad en Uruguay. Conducta perinatal del equipo de salud. Opinión de expertos

Perinatal viability in Uruguay. Perinatal actions by the health team. The opinion of experts

RESUMEN

En Uruguay no se conoce la opinión de los profesionales en relación a cuál sería para ellos la edad límite de viabilidad humana ni su conducta en general cuando se enfrentan a prematuros extremos, motivo por el cual se implementó la encuesta.
Se realizó un trabajo descriptivo, observacional y de corte transversal.
De los 83 encuestados casi 90% o más encuentra muy importante el rol que cumple la familia como participante activo en la toma de decisiones junto al equipo de salud.
Las acciones terapéuticas tomadas a favor de proporcionar un tratamiento proactivo para brindarle posibilidades de sobrevida al recién nacido (inicio de corticoïdes, antibióticos profilácticos, útero-inhibición, monitorización de bienestar fetal, derivación a CTI neonatal, etcétera) se hacen más manifiestas a partir de las 24 semanas de edad gestacional en adelante con un porcentaje mayor a 90% en todas las preguntas referidas.
Más de 90% de los médicos neonatólogos y hasta 50% de los médicos ginecólogos no se siente amparado por un marco legal que garantice sus acciones dentro de lo ético y humanamente justificable.
Como conclusión de este trabajo las opiniones manifestadas por los profesionales encuestados es que la viabilidad neonatal se encuentra entre las 24 y 26 semanas, diferenciándose con la franja de edades gestacionales que debaten en la actualidad distintas Sociedades Científicas Internacionales en la materia, muchas de las cuales encuentran su límite entre las 23 a 24 semanas.

ABSTRACT

The opinion of professionals in relation to what they understand the age limit for human viability is, or the actions they take in general, when they face premature ends are not known in Uruguay. For this reason, a survey was implemented to learn about this issue. A descriptive, observational and cross-sectional study was conducted.

Almost 90% or even more of the 83 physicians consulted find the role of the family is very important as they are active participants in the making of decisions along with the health team. The therapeutic actions taken geared to providing a proactive treatment to offer the newborn the possibility of survival (initiation of corticoids, prophylactic antibiotics, utero inhibition, monitoring of fetal well-being, and referral to neonatal ICU, etc.) are more obvious as from 24 weeks of gestational age, being it over 90% in all questions referred. Over 90% of neonatologists and up to 50% of gynecologists believe they are acting without a legal framework that guarantees their actions in terms of what is ethically and humanly justifiable. As a conclusion of this study, the opinions by the professionals surveyed is that neonatal viability is between 24 and 26 weeks, differing from the strip of gestational ages international scientific societies debate nowadays, many of which finds the boundary between 23 to 24 weeks.

Impacto de la implementación de oxigenoterapia de alto flujo en el manejo de la insuficiencia respiratoria por infecciones respiratorias agudas bajas en un departamento de emergencia pediátrica

Impact of high flow nasal cannula oxygen in the management of acute respiratory failure in a Pediatric Emergency Department

RESUMEN

Introducción: La oxigenoterapia de alto flujo (OAF) administrada por cánulas nasales, se ha instaurado como una técnica sencilla, fácil de administrar, de bajo costo, sin complicaciones graves, efectiva para el tratamiento de la insuficiencia respiratoria (IR) en infecciones respiratorias agudas bajas (IRAB). Su aplicación temprana podría mejorar la evolución de estos niños.
Objetivos: Comunicar la primera experiencia con OAF en niños con IRAB en un Departamento de Emergencia Pediátrica (DEP). Compararla con una cohorte histórica de niños que no la recibió. Métodos: Estudio descriptivo, prospectivo (1 de junio de 201320 de setiembre de 2013). Todos los niños tratados con OAF en DEP del Centro Hospitalario Pereira Rossell. Criterios de inclusión: <2 años con IRAB viral con IR y escore de Tal >8 o >7 mantenido, apneas reiteradas, saturación de oxígeno <90% con O2 por máscara de flujo libre. Criterios de exclusión: pCO2 >70 mmHg, pH <7,2, depresión de conciencia, falla hemodinámica.
Resultados: OAF 36 niños; mediana 4 meses; bronquiolitis 83%; VRS+ 58%. Destino pacientes en OAF: cuidados moderados 78%, UCI 22%, AVM 22%. No complicaciones ni fallecimientos. Cohorte histórica: 91 niños con IRAB no tratados con OAF. Cohorte histórica: UCI: 40 (44%) versus OAF (p= 0,0005). AVM: cohorte histórica 30 (33%) versus OAF (p= 0,026). Menores 6 meses: con OAF AVM 5 (19%), cohorte histórica: 25(45%) (p= 0,026).
Conclusiones: En un porcentaje elevado de pacientes fue posible evitar el ingreso a UCI. La necesidad de AVM en menores de 6 meses con OAF fue significativamente menor. La incorporación temprana de OAF en las IRAB graves modificó la forma de tratamiento de estos pacientes en la emergencia.

ABSTRACT

Introduction: High flow nasal cannula oxygen (HFNC) has become a simple, easy to implement, non-expensive technique, without serious complications, to treat respiratory failure in lower respiratory tract infections (LRTI). Its early implementation could improve outcome in these children.
Objectives: To communicate the first experience with HFNC in children with LRTI in a Pediatric Emergency Department. To compare it with a historical cohort of children who did not receive HFNC.
Methods: Descriptive, prospective study (01/06/13-20/09/2013). Every child treated with HFNC in the Emergency Department -Pereira Rossell Hospital. Inclusion criteria: <2 years old with viral LRTI, respiratory failure and Tal score >8 or persistent > 7, persistent apnea, oxygen saturation <90% with O2 mask. Exclusion criteria: pCÜ2 >70 mmHg, pH <7.2, depression of consciousness, hemodynamic failure.
Results: HFNC 36 children; median age 4 months; bronchiolitis 83%; VRS+ 58%. Destination HFNC patients: admission 78%, PICU 22%, MV 22%. No complications, no deaths. Historic cohort: 91 children with LRTI not treated with HFNC; PICU: 40(44%) vs. HFNC (p=0.0005). MV: historic cohort 30(33%) vs. HFNC (p=0.026). Younger than 6 months: HFNCO MV 5(19%), historic cohort: 25(45%) (p=0.026).
Conclusions: Admission to PICU was prevented in a high number of patients. Need for MV in patients younger than 6 months with HFNC was significantly lower. The early application of HFNC in severe LRTI modified the treatment of these patients in the emergency department.

RESUMEN

Los adenovirus humanos (HAdV) causan una diversidad de patologías que pueden ser leves o incluso mortales en pacientes vulnerables. Su evolución ocurre conforme actúa una presión de selección; por ejemplo, la recombinación genética genera cepas híbridas que pueden ser más infectivas o más patogénicas que las parentales. Este estudio buscó determinar la diversidad de HAdV circulantes en niños paraguayos menores de 5 años hospitalizados por infección respiratoria aguda baja, mediante el uso de herramientas de biología molecular y bioinformática, para lo cual fueron amplificados los genes hexon, penton y fibra en 26 muestras de aspirados nasofaríngeos. Se demostró la circulación de tres especies: B (4/26), C (21/26), y D (1/26), así como también eventos de recombinación en el aislado correspondiente a HAdV-D, al menos con tres genotipos distintos (D49, D9 y D15). Los HAdV-D están asociados a enfermedades gastrointestinales, oculares y, menos comúnmente a infecciones respiratorias. Sin embargo, recientemente se han reportado cambios en el comportamiento de estos virus debido a recombinación, lo que les permite expandir su repertorio de células huésped. Estos datos amplían los conocimientos acerca de la diversidad genética de HAdV en el Paraguay y apoyan fuertemente la importancia de los análisis genómicos mediante herramientas bioinformáticas en la vigilancia epidemiológica de las cepas recombinantes emergentes de HAdV.

ABSTRACT

Adenoviruses cause a variety of diseases that range from mild to fatal in vulnerable patients. Its evolution occurs as a process of selection pressure; for example, genetic recombination generates intermediate strains that can be more infectious or pathogenic than parental strains. This study sought to determine the diversity of circulating adenoviruses in Paraguayan children less than 5 years of age hospitalized with acute lower respiratory infection, by using molecular biology and bioinformatics tools, though amplification of penton, hexon and fiber genes in 26 samples of nasopharyngeal aspirates. It was demonstrated the circulation of three species: B (4/26), C (21/26), and D (1/26), as well as recombination events corresponding to the HAdV-D isolate, with at least three different genotypes (D49, D9 y D15). The HAdV-D are associated with gastrointestinal or ocular diseases, and less commonly with respiratory infections. Recently, however, it has been reported changes in behavior of these viruses due to recombination, allowing them to expand their host cell repertoire. These data extend the knowledge about the genetic diversity of HAdV in Paraguay and strongly support the importance of genomic analysis using bioinformatics tools in epidemiological surveillance of emerging recombinant strains of HAdV.

Tratamiento con solución salina hipertónica en lactantes hospitalizados con bronquiolitis aguda

Treatment with hypertonic saline in hospitalized infants with acute bronchiolitis

RESUMEN

Introducción. El edema de las vías aéreas y las secreciones mucosas son características patológicas predominantes en lactantes con bronquiolitis aguda (BA). La nebulización con solución salina hipertónica al 3% (SSH) puede reducir los cambios patológicos y disminuir la obstrucción de la vía aérea.
Objetivos. Evaluar en lactantes con bronquiolitis aguda la eficacia del uso de SSH para disminuir días de internación y mejoría en el score clínico (SC).
Material y métodos: Un total de 106 pacientes (edad media de 5,1 ± 6,4 meses) (rango: 1-14 meses) fueron admitidos y reclutados para recibir tratamiento de BA, en tres hospitales: Cátedra de Pediatría del Hospital de Clínicas, Hospital Barrio Obrero, Instituto Privado del Niño. Los pacientes fueron internados en sala de internación común. El periodo de estudio fue de mayo del 2011 a julio del 2014. Se realizó un ensayo clínico, aleatorizado, controlado y a doble ciego, divididos en 2 grupos para recibir solución salina hipertónica (SSH) o solución salina normal (SSN) con epinefrina.
Resultados. Los lactantes del grupo SSH tuvieron importante disminución en los días de hospitalización 1,7±0,76 vs 4,9±2,3 del grupo SSN, p<0,001. El SC demostró mejoría significante en el grupo SSH comparando con el grupo SSN (p<0,001).
Conclusiones. La solución salina al 3% con epinefrina es más efectiva que la solución salina normal con epinefrina en pacientes moderadamente severos con bronquiolitis aguda.

ABSTRACT

Introduction. Airway edema and mucous secretions are predominant pathological features in infants with acute bronchiolitis (AB). Nebulization with hypertonic saline at 3% (HSS) can reduce the pathological changes and decrease airway obstruction.
Objectives. To evaluate the effectiveness of using HSS in infants with acute bronchiolitis to decrease hospital length-of-stay and improvement in the clinical score (CS).
Material and methods. A total of 106 patients (mean age of 5.1 ± 6.4 months) (range: 1-14 months) were admitted and enrolled for AB treatment in three hospitals: University Hospital, Barrio Obrero Hospital, and the Private Children's Institute. Patients were admitted to regular hospital rooms. The study period was from May 2011 to July 2014. We performed a randomized, controlled, double-blinded clinical trial, dividing the subjects into 2 groups: one to receive hypertonic saline (HSS) or normal saline (NS) with epinephrine.
Results. Infants in the HSS group had a significant decrease in hospital length-of-stay days, 1.7 ± 0.76 vs. 4.9 ± 2.3 in the NS group, p <0.001. The CS showed significant improvement in the HSS group compared to the NS group, p < 0.001. Conclusion. 3% saline with epinephrine is more effective than normal saline plus epinephrine in patients with moderately severe acute bronchiolitis.
