Gradiente extremo en miocardiopatía hipertrófica

Extremely severe gradient in hypertrophic cardiomyopathy

La imagen corresponde a un paciente de 73 años, de sexo masculino, tabaquista importante de larga data e hipertenso, sin otros antecedentes relevantes, que concurrió para la realización de un eco-Doppler cardíaco por presentar en el examen físico un soplo sistólico 2/6 en foco aórtico, con un diagnóstico presuntivo de estenosis valvular aórtica. Se encontraba asintomático. En el estudio se halló que el paciente presentaba una miocardiopatía hipertrófica parcialmente obstructiva al tracto de salida del ventrículo izquierdo, con registros de velocidad pico sistólica mayor de 7 metros por segundo y gradiente intraventricular mayor de 200 mm Hg.
